To register for class call 233-1598 for availability, fill out this form, pay
a $15.00 deposit by pay pal or mail payment in full to Lynne Fischer at 107 Spring Hollow Lane, Cary NC 27518. I
look forward to teaching you a fun class!

This $15.00 fee is a deposit and it is required to hold your place in the class. The remainder is due at the first
class. Deposits are not refundable.

By submitting this form I give permission for myself or my child to participate
in the class offered by Lynne Fischer at the Art Adventures Studio. I agree for myself, or my child to assume full responsiblility
for and hold Lynne Fischer harmless from any legal liability, injury or damage to person or property in connection with the
participation in this class.

In the event the class must be cancelled, due to weather or any unforseen reason, a
makeup date will be posted. I follow the Wake County School system for closings. If a student has to miss a class I will catch
the student up with either homework or extra class time.
